Focus Awards Replacement Certificates
If your certificate has been lost or damaged, please email info@focusawards.org.uk to obtain a replacement. Replacement incurs an administration fee of £30, which includes processing and posting. Replacement certificates will indicate that they are a replacement of the original.

    Proof of identity applications will be delayed unless a photocopy of one of the documents below accompanies the form.
    Please indicate which documentation has been forwarded with this form by ticking the appropriate box.
    All copies must be clear, show name and date of birth.

    Please do not send original documents with your applications as we cannot guarantee their safe return. Once your application has been processed and completed, the documents supplied will be securely destroyed.
